Convenient, Specialized Daytime Memory Care Program

The Bridge | Adult Day Care Services

Our exceptional Adult Day Services Program is unlike a traditional Adult Day Care program. As a memory care community, Monticello West has been specializing in dementia care for more than 36 years. We are proud of our history of offering personalized care to our residents and we are excited to offer this same level of care to our guests who participate in our Adult Day Services program called The Bridge.

 

We offer a safe and welcoming environment where your loved one can connect with others and enjoy enriching daily activities. The program is uniquely designed for those with Alzheimer’s disease, dementia or related conditions who need daytime support.

Who We Serve

Our daytime program is an ideal solution for seniors who still live in the family home but who could benefit from cognitive, social and physical stimulation during the day. To best benefit from our program, we recommend members:

• Have a diagnosis of Alzheimer’s disease or dementia or have other circumstances that would make them well-suited for a memory care environment
• Must be at least 55 years old
• Are in a stage of disease progression that still allows for active participation in program activities

Extended Hours, 7 Days a Week

Unlike traditional Adult Day Care programs that offer limited weekday hours, our Adult Day Service Program offers you convenient hours, so you have access to daytime services when you need them. Our program is open 7 days a week from 7:30 a.m. to 7:30 p.m. You can customize the number of days to fit your needs, with a minimum of two days per week.

Hours of Operation
Sunday – Saturday
7:30 a.m. – 7:30 p.m.

Specialized Cognitive and Physical Programming

Our cognitive and physical programming provides ample opportunities for your loved one to stay engaged throughout the day.

We maintain a staff ratio of 1:5 so that our staff can truly get to know your loved one and his or her interests. Our staff uses principals from our exclusive Heartfelt CONNECTIONS™ – A Memory Care Program® to connect on a personal level and focus on meaningful, purposeful moments with each member.

We provide numerous services to ensure the safety and well-being of each member. Our goal is to provide your loved one with individualized care and dignity during his or her time with us.

Meal Services

All meals and snacks prepared on-site and include:

• Continental breakfast
• Morning snacks
• Appetizing, hot nutritional lunch served restaurant style
• Assistance with meals, including prompts/cues, cutting up meats and adaptive equipment
• Supervision and assistance for those with choking precautions
• Hydration program – juices and water served at least three times daily
• Dinner available from 5-6 p.m.

Health and Well-Being Services

Our Adult Day Services program is designed to provide the support your loved one needs depending on his or her individual condition. Services include:

• Toileting reminders
• Assistance with toileting/incontinence care
• Assistance with transfers and/or ambulation
• Assistance and monitoring of oxygen
• Quiet room for calming
• Private room available for rest if needed
• Glucose testing*
• Nurse available during the day as needed

Personal Care Services*

For an extra charge, we offer the following personal care services:

• Showers
• Men’s shaves
• Hair shampoo and blow dry
• Professional, licensed salon services for both ladies and men

Family Support Services

We want our families to feel involved in the daily activities of their loved ones. To support families, we provide:

• Monthly family support education
• Video check-in for family every afternoon

Purpose and Connections Through Meaningful Activities

Our day program includes a variety of activities to fill your loved one’s day with purpose. Through daily activities, members have the opportunity to engage in hobbies that are familiar to them or that they have enjoyed in the past. Activities include:

• Men’s and ladies programming based on interests
• Word and trivia games
• Dancing with live entertainment
• Musical programs
• Animal therapy
• Intergenerational programs
• Exercises twice daily
• Arts and crafts
• Mind fitness activities
• Field trips

Customize Our Daytime Program to Meet Your Loved One’s Needs

We look forward to sharing more about our Adult Day Services Program with you. Before enrollment, please read our program policies [link to PDF] to make sure your loved one is a good fit for our program. Then, follow these steps to enroll:

• Schedule a tour to learn more about our program
• Complete the Adult Day Services Program application [link to PDF]
• Schedule an assessment – We use assessments to develop a care plan unique to each member. Assessments are completed within one to two business days. Member may begin using services within one business day of assessment completed and all application materials submitted and acceptance from Monticello West received.
• Reserve day services – Reservations can be submitted Monday through Friday.
